List of Documents It Will Generate

CLIENT LETTERS

Client AA: Blank letter to the client, input text of your choice.

Client AB: Letter to the client stating that you have not been able to contact him/her by phone, requests that client contact your office.

Client AC: Letter asking client for more information about potential Social Security benefits, includes request for information to be sent by Social Security.

Client AD: Letter asking client to authorize review of Social Security and medical files, includes request for information from Social Security and authorization to review client’s medical records.

Client AE: Second request for authorization to review Social Security and medical records.

Client AF: Letter to confirm the client’s first appointment, contains information on what to bring to the appointment.

Client AG: Letter reminding client of date and time of first appointment, as well as what to bring.

Client AH: Letter notifying client that first appointment was missed, requests that they call to reschedule.

Client AI: Letter confirming rescheduling of first appointment, explains what to bring.

Client BC: Letter stating that your organization will not be able to represent the client, due to his/her case being at the reconsideration stage.

Client BD: Letter declining representation because Title II benefits are too high for a client who has been initially denied benefits (for a firm representing indigent clients).

Physician FE: Letter requesting a complete medical assessment of client (private firm).
